About this file

This is a Proposal Cover Sheet for a Mini-RFP issued by the Oregon Department of Transportation for CA/CEI (Construction Administration/Construction Engineering and Inspection) Services related to the Region 3 Southwest Oregon ADA Curb Ramps project (2024-2027). The project encompasses approximately 380 curb ramps distributed across various cities in Douglas, Coos, and Curry Counties in southwest Oregon. The solicitation (Mini-RFP/Requisition #S-73000-00017466) seeks proposals from eligible Price Agreement holders to provide services during the construction phase. Proposers must hold a professional engineer designation as a minimum requirement and must submit their proposal using this cover sheet format, which serves as the official submission document for the evaluation and selection process.

The cover sheet requires proposers to certify compliance with multiple state requirements, including non-discrimination practices toward minority and women-owned enterprises, sexual harassment prevention policies (mandatory for contracts valued at $150,000 or more), and conflict of interest disclosure obligations. Proposers must confirm receipt of all addenda, affirm the truthfulness of proposal contents, declare absence of undisclosed delinquent debt to the state, and accept all terms and conditions contained in the Mini-RFP and referenced Professional Agreement. All proposals must be authorized and executed by a duly authorized representative of the proposing firm. The cover sheet does not specify pricing terms, funding amounts, response deadlines, or contract duration details within this particular document.
