About this file

This memorandum documents the results of Request for Quote (RFQ) 36C24825Q0021 for Custom Surgical Packs for the Orlando VA Medical Center. The solicitation was set aside 100% for service-disabled veteran-owned small businesses (SDVOSB) and was originally posted on SAM.gov on December 26, 2024, with multiple amendments extending the offer due date to March 5, 2025.

Contracting evaluated quotes from five different SDVOSBs (Howds Hill LLC, Manus Medical LLC, AlphaVets Inc., Veterans Healthcare Supply Solutions, and Geo-Med LLC), but all offerors failed the administrative review due to incomplete or unsigned documentation related to limitations on subcontracting. As a result, no award was made, and Contracting intends to resolicit the requirement as an unrestricted procurement on the open market. The RFQ was effectively canceled on March 10, 2025, by the Contracting Officer from NCO 8 – Orlando Commodities and Services.

To: Contracting Officer

On December 26, 2024, Contracting posted a Request for Quote (RFQ) (36C24825Q0021) on SAM.gov for Custom Surgical Packs for the Orlando VA Medical Center in Orlando, FL. This requirement was set aside 100% to service-disabled veteran-owned small business (SDVOSB). Questions were due no later than 01/22/2025 at 12:00 PM EST and offers were due no later than 02/05/2025 at 3:00 PM EST. The following Amendments were also issued:

01/28/2025 Amendment 0001 was posted on SAM.gov to extend the offer due date to 02/14/2025 at 3:00 PM EST and to answer questions.

02/05/2025 Amendment 0002 was posted on SAM.gov to reissue VAAR 852.219-76 VA

NOTICE OF LIMITATIONS ON SUBCONTRACTING – CERTIFICATE OF

COMPLIANCE FOR SUPPLIES AND PRODUCTS (NOV 2022) with revisions.

02/13/2025 Amendment 0003 was posted on SAM.gov to revise the Salient Characteristics, reopen the Question and Answer Period, and extend the offer due date. The Question and Answer period was reopened until 02/20/2025 at 3:00 PM EST and the offer due date was extended to 03/05/2025 at 3:00 PM EST.

02/25/2025 Amendment 0004 was posted on SAM.gov to answer questions.

Following the closure of the solicitation period on March 5, 2025, at 3:00 PM EST, Contracting evaluated quotes from five (5) different SDVOSBs.

Howds Hill LLC (SDVOSB) submitted a quote on February 5, 2025. The quote did not include a total estimated price and also did not include any signed Amendments as requested. The offeror fill-ins for VAAR 852.219-76(a)(1)(i) and (ii) were incomplete and the quote did not include pictures of surgical pack components as requested. There was no proof this offeror could comply with the limitations on subcontracting. As a result, this offeror failed Contracting’s administrative review.

Manus Medical, LLC (SDVOSB) submitted a quote on March 4, 2025. Amendments 0001-0004 were signed, however the offeror fill-ins for VAAR 852.219-76(a)(1)(i) and (ii) were incomplete. There was no proof that this offeror could comply with the limitations on subcontracting. As a result, this offeror failed Contracting’s administrative review.

AlphaVets, Inc. (SDVOSB) submitted a quote on March 5, 2025. Amendments 0001-0004 were signed, however the offeror did not sign the latest version of VAAR 852.219-76 issued by Contracting on 02/05/2025. As a result, this offeror failed Contracting’s administrative review.

Veterans Healthcare Supply Solutions (VHSS) (SDVOSB) submitted a quote on March 5, 2025. The offeror stated in its email that they would be unable to sign the solicitation document as well as Amendment 0002 because of the Limitations on Subcontracting set forth in VAAR 852.219-76. As a result, this offeror failed Contracting’s administrative review.

Geo-Med, LLC submitted a quote on March 5, 2025. The offeror stated in its email that they would be unable to sign the solicitation document as well as Amendment 0002 because of the Limitations on Subcontracting set forth in VAAR 852.219-76. As a result, this offeror failed Contracting’s administrative review.

Failure to meet limitations on subcontracting made all SDVOSB offerors ineligible for award.

Contracting intends to resolicit this requirement as unrestricted on the open market.

Therefore, no award is being made from RFQ 36C24825Q0021 and the RFQ is effectively canceled.
