About this file

This is a Questions and Answers document for the Civil Money Penalty Quality Improvement Initiative Grant Program issued by the State of Iowa Department of Health and Human Services, Medicaid Division. The RFP solicits grant proposals to fund quality improvement projects in federally certified nursing facilities using civil money penalties collected from nursing home violations. Eligible project categories include resident and family councils, consumer information development, training to improve quality of care, activities to improve quality of life, mental and behavioral health services, and workforce enhancement initiatives. Written questions were due by April 29, 2026, with proposal responses due by May 29, 2026. Project selection is scheduled for July 22, 2026, with award announcements by September 22, 2026, contract execution by November 2, 2026, and project start dates of January 1, 2027. Contract terms are one year extendable up to two additional one-year periods, for a maximum of three years.

Funding is derived from the Iowa CMP fund balance of $21,298,291.96 as of December 31, 2025. Maximum funding amounts are category-specific, ranging from $6,000 per nursing home for one-time resident and family council or consumer information projects to $18,000 maximum for three-year projects. Most training and quality of life improvement categories are capped at $6,000 per nursing home per year, with multi-year projects accumulating up to $18,000 over three years. The funding cap applies per facility per year, with no maximum limit on the number of nursing facilities that can participate in a single project. Applicants must submit hard copy responses with a flash drive to the issuing officer; email submissions are not accepted. A current list of facilities actively participating in CMP-funded projects and therefore ineligible is maintained and made available to applicants. All awards are subject to final approval by the Centers for Medicare and Medicaid Services, and payment is made quarterly in arrears based on invoices documenting services rendered and compliance with deliverables and performance measures.
