Subject: Request for Quotation (RFQ) – Medical Office Shoes RFQMD030415
To potential bidders:
THIS IS A SUBCONTRACTING OPPORTUNITY with Management & Training Corporation (MTC), operator of the Inland Empire Job Corps Center. MTC is requesting bids for a soil resistant white shoe that may be suitable for use by trainees in the Medical Office profession. This shoe should be a step above standard tennis shoe. Please provide a price quote including any shipping and handling for the attached listing Exhibit 1 (it is understood the manufacturer may change part numbers). Inland Empire Job Corps Center is seeking to standardize the style and type of shoe in order to allow for repeat orders. The goal of this shoe is twofold: be comfortable for trainees to wear to work in a semi-professional (office) environment and not look like a tennis shoe. The shoe bid must be a light weight casual slip-on style, with a slip resistant rubber outsole with superior grip, a light weight shock absorbing composite foam mid-sole for extra comfort, and a polyurethane footbed to reduce fatigue. The shoe should also shield and control odor, staining and deterioration. This shoe shall be oil and water resistant and ideally suited for professionals who tend to stand all day on hard surfaces, possibly with water or oil such as chefs, doctors, teachers, and retail store or restaurant employees. Bidder shall provide a photo or other suitable picture of the shoe proposed.
Delivery terms and an estimated delivery date must also be specified.
Items shall be shipped to:
Inland Empire Job Corps Center 3173 Kerry St San Bernardino CA 92407
MTC Terms and Conditions:
Management & Training Corporation (MTC) Terms and Conditions are included with this RFQ, and can be found at www.mtctrains.com. It is the Contractor’s responsibility to verify MTC’s current Terms and Conditions at the time services and/or goods are provided.

Inland Empire Job Corps Center is exempt from the assessment of sales tax for the purchase of tangible property or services under the provisions of the Workforce Investment Act, U.S. Public Law 105-220 dated August 7, 1998.

Reported information will be made public via the FFATA Sub-award Reporting System (FSRS.gov).

NO FAXED PROPOSALS ACCEPTED
MTC reserves the right to reject any or all bids or waive any formality connected with the bids. MTC will be the sole judge of which bid is the best value and its determination and finding on such decision will be final.
