About this file

This specifications document outlines a Request for Quotations (RFQ) issued by the West Virginia Office of Shared Administration, Purchasing Division, on behalf of the Department of Health Facilities for Welch Community Hospital to establish an open-end contract for the rental of cylinders and purchase of industrial and medical gases. The contract encompasses the provision of eight gas types—Acetylene, Air, Argon, Carbon Dioxide, Helium, Oxygen, Nitrogen, Nitrous Oxide, and Diffusion—across 17 specific cylinder configurations ranging from industrial large to medium x-small sizes, plus hazardous materials shipping charges. Filled tanks are to be delivered weekly with empty cylinders collected simultaneously; standard orders require bi-weekly delivery with emergency orders fulfilled within 24 hours. The vendor must ensure all cylinders comply with Department of Transportation regulations, inspect cylinders upon delivery, and maintain sufficient inventory to meet contractual obligations. No specific response date, bid due date, site visit requirement, bidder conference, or award date is identified in this document; the contract term and renewal options are similarly not specified.

The contract will be awarded to the vendor submitting the lowest grand total cost for all itemized contract services. Vendors must complete comprehensive pricing pages detailing unit costs, annual costs, and grand total costs for each item, with failure to complete pricing pages resulting in disqualification. The contract incorporates federal funding and requires vendor compliance with the Federal Funds Addendum and Health Insurance Portability and Accountability Act (HIPAA) Business Associate Agreement provisions. Standard order delivery is F.O.B. destination with delivery costs included in bid pricing; emergency order delivery charges are billable separately. The vendor must designate a primary contract manager available during business hours, provide quarterly and annual purchase reports, accept orders via mail, facsimile, email, or optional secure online ordering systems, and accept payment according to West Virginia state procedures.
