About this file

This document is a Bidders Submission Checklist for RFR MED52, which is a state contract opportunity for Influenza Vaccine for the 2018 - 2019 Flu Season issued by the Commonwealth of Massachusetts. The checklist outlines the required forms and documents that bidders must complete and submit as part of their response, including the Bidders Price Sheet, Prompt Payment Discount Form, Supplier Diversity Program Plan Form, and Certificate of Insurance. The contract term is for the 2018 - 2019 flu season.

The pricing terms, funding requirements, and any set-aside information are not explicitly detailed in this checklist document. However, it does note that bidders must submit a signed statement from each manufacturer they are bidding on, stating the manufacturer will supply the bidder with the proposed quantities for the Commonwealth.
