STATEMENT OF WORK
FOR FACILITY ACCESS CONTROL PROGRAM AND ID BADGE CARD READER SYSTEM REPLACEMENT
56TH MEDICAL GROUP
BLDG 1130, 1135, 1146 & 1514

The 56th Medical Group (MDG) has a requirement to replace their facility access control program and ID badge card reader system. The current system is outdated and the software is unable to be supported.
The system lacks capability to support additional badge readers. The MDG requests to host an on-site visit with all prospective contractors prior to contract award.
I. Project Duration: Once Notice to Proceed is granted, project is to be completed within 30 calendar days. Extensions will be granted for unforeseen conditions, and other factors outside of the contractor’s control.
II. Project Scope:
A. Contractor is to provide all labor, materials, equipment and disposal necessary to replace the current facility access control program and ID badge card reader system. This project shall be executed in such a way as to avoid impacting the Clinic operations.
B. Work areas around project work sites will be occupied during the course of work. Caution must be taken to ensure all safety work practices are adhered to while the project is being performed around the facility. Contractor shall be responsible to ensure adequate dust control. Work will be conducted with minimal to zero impact to patient care. The project site shall be kept in a neat, orderly and safe condition at all times.
C. Caution must be taken in all work areas at the facility to avoid any damage to existing government property. Any damage to government property resulting from work being performed shall be the responsibility of the contractor to restore to like new condition.
D. All work and material shall comply with the most current Luke AFB Facility Design Standard, UFC, International Building Code, NFPA, NEC and industry standards that apply to this Scope of Work. The contractor shall ensure that employees and sub-contractors have sufficient knowledge, experience and qualifications necessary to provide the required level of service.
1) Contractor shall provide and install a new facility access control program and ID badge card reader system, to include:
a) New computer with multi-user software providing database administration, event monitoring and reporting of system history and cardholder records
b) 36 controllers with option to increase the number of controller/badge readers
c) 11 large tamper resistant enclosures to house controllers
d) Install 3 badge reader (Command Section, Housekeeping, building 1141)
e) Color PVC printer/ribbon
f) Compatible HD camera with tri-pod stand
g) Emergency lockdown and release button
h) Capability to create lockdown override badges for emergency responders
i) On-site initial system training
j) Any applicable licenses and warranties
2) Contractor is to provide a maintenance plan which will include the following:
a) Tech support during 56 MDG business hours (0730-1630, Monday through Friday)
b) On site response, 6 hours or less from the time the service call is placed
c) Preventative maintenance, to include quarterly inspections of hardware/software and database maintenance
d) All software updates as available on stand-alone system
e) Contractor will mitigate hardware/software down time with temporary replacement equipment
3) Contractor is to follow all base requirements and applicable codes for installation and repair.
4) During installation, Contractor must ensure that all 56 MDG buildings are able to be secured and locked at the end of the duty day.
5) Contractor will ensure that all material and equipment are neat and professional in appearance and do not interfere with work areas.
III. Submittals: After completion of all work, the Contractor shall submit to the Facility Manager the manufacturer's specifications, instructions and material specification sheets in original form.
IV. Contractor Use of the Premises: During the period of the project, the contractor will have access to the site only during normal working hours: Monday through Friday, 0730-1630. No on-site work shall be performed outside these hours or on federal holidays and clinic down days.
V. Cleanup/Material Removal and Disposal: Before scheduling final inspection, work areas shall be thoroughly cleaned and ready for use. Contractor shall remove all tools, equipment, surplus material and rubbish. Restore or refinish, to original condition, surfaces that are damaged due to the work of this contract. Remove grease, dirt, stains, foreign materials and labels from finished surfaces. The contractor is responsible for removal of all demolition materials and/or excess materials from the project.
VI. Safety & Security:
A. Contractor shall follow existing security procedures and instructions applicable to Luke AFB.
B. Contractor shall conform to all safety requirements.
C. Contractor shall be responsible for the physical security of all materials, supplies and equipment, including property utilized.
