About this file
The document is a Living Wage Certification Form from the Town of Manchester, Connecticut, related to a contract opportunity for Chemicals II for Wastewater and Water Treatment. The form requires bidders to indicate whether they are a "Covered Employer" under the Manchester Living Wage Ordinance (Chapter 212 of the Manchester Code of Ordinances) and comply with its requirements. Bidders must certify their status by either affirming they will pay the required living wage to eligible employees throughout the contract term or by claiming an exemption based on specific criteria such as being a nonprofit organization, employing fewer than 25 eligible employees, or having an annual contract value less than $25,000.
The form is designed to ensure fair compensation practices for workers involved in municipal contracts, with potential bidders required to complete and submit this certification as part of their bid or proposal package. Failure to properly indicate employer status may result in bid rejection. The document provides checkboxes for different exemption categories, including charitable foundations with 501(c) tax-exempt status, small employers, and contracts below a certain monetary threshold. The certification must be signed by an authorized representative of the bidding company, attesting to the accuracy of the information provided.
