About this file

The document is a Living Wage Certification Form issued by the Town of Manchester, Connecticut, related to a municipal contract opportunity for infield clay mix. The form requires bidders to certify whether they are a "Covered Employer" under the Manchester Living Wage Ordinance (Chapter 212 of the Manchester Code of Ordinances), and if so, to commit to paying the required living wage to eligible employees throughout the contract term. Bidders must indicate their status by checking one of two primary options: either confirming they are a covered employer who will comply with wage requirements, or declaring an exemption based on specific criteria such as being a nonprofit, employing fewer than 25 eligible employees, or having an annual contract value under $25,000.

The certification form is designed to ensure contractor compliance with local wage standards and must be completed and returned with the bid or RFP submission. Failure to properly indicate the employer's status may result in bid rejection. The form provides multiple exemption pathways, including nonprofit status (specifically those exempt from federal income taxation under Section 501 of the Internal Revenue Code), small employer size, and low contract value. The document requires the bidder's authorized representative to sign and date the form, certifying the accuracy of the representations made regarding their employment and wage practices.
