About this file
The document is a Living Wage Certification Form issued by the Town of Manchester, Connecticut, requiring bidders to certify their compliance with the Manchester Living Wage Ordinance (Chapter 212 of the Manchester Code of Ordinances). The form is specifically designed to be submitted with a bid or RFP submission for the 942 Main Street Development project. Bidders must indicate whether they are a "Covered Employer" subject to the living wage requirements or qualify for an exemption based on specific criteria, such as being a nonprofit organization, employing fewer than 25 eligible employees, or having an annual contract value less than $25,000.
The certification form mandates that covered employers pay the required living wage to eligible employees and comply with the ordinance throughout the contract term. Failure to complete the form or indicate the appropriate status may result in bid rejection. The form provides three potential exemption categories: charitable foundations or nonprofits with federal tax-exempt status, employers with fewer than 25 eligible employees, and contracts with an annual value below $25,000. The document requires the authorized representative to sign and date the form, certifying the accuracy of the representations made regarding the bidder's employment and wage practices.
