About this file
The document is a Living Wage Certification Form issued by the Town of Manchester, Connecticut, pertaining to the Manchester Living Wage Ordinance (Chapter 212 of the Manchester Code of Ordinances). This form is specifically associated with a material testing services and special inspection contract for the Bowers Elementary School renovations (State Project #077-0239 RNV). Bidders are required to complete this form indicating whether they are a "Covered Employer" subject to the living wage requirements or qualify for an exemption.
The form provides three potential exemption categories: (1) charitable foundations or nonprofits exempt from federal income taxation, (2) bidders employing fewer than 25 eligible employees, or (3) contracts with an annual value less than $25,000. Bidders must accurately complete and submit this certification with their bid or RFP submission, with a clear warning that failure to indicate their status may result in bid rejection. The form requires an authorized representative to sign and certify the accuracy of the representations made, linking the living wage compliance to the specific bid or RFP for the Bowers Elementary School renovation project.
