About this file

This document is a Past Performance Questionnaire (PPQ) for the MACPAC Survey Data Analysis Task Order Contract (Solicitation No. 202502) for Fiscal Years 2026-2035. The PPQ is designed to evaluate potential contractors' past performance across three key areas: technical performance and quality, management approach, and schedule adherence. The questionnaire uses a performance rating scale from Exceptional (E) to Unsatisfactory (U), with evaluators required to provide detailed comments for ratings at or below Satisfactory.

The associated solicitation seeks a contractor to perform survey data analysis and technical assistance for MACPAC, with specific requirements including a pool of statistical programmers, expertise in survey design and methodology, proficiency in programming languages like SAS and STATA, and ability to analyze complex survey data. The solicitation is open to all sources, with a NAICS code of 541720. Key dates include a question submission deadline of May 19, 2025, response posting by May 23, 2025, and proposal submission deadline of June 6, 2025 at 5:00 PM Eastern Time. Prospective contractors must be registered in the System for Award Management (SAM) database prior to contract award.
