PAST PERFORMANCE QUESTIONNAIRE

Instructions:

· Sections I through III to be completed by the Offeror/Major Subcontractor and provided to the assessor (Government personnel who had experience with the Offeror/Major Subcontractor on a previous relevant contract). The assessor, in turn, will verify Sections I through III, making corrections as necessary; complete the questionnaire; and submit it to the Contracting Office two (2) days prior to the close of the solicitation on 14 July 2016.

· Sections IV through VII to be completed in their entirety by the assessor.

Message to the assessor: Your assistance is requested by the USMC to assist with establishing the performance history for the Contractor named below. In efforts to expedite receipt of the requested information, the Contracting Office respectfully requests that you do not mail hard copies. Project/Requirement Description
The main objective of this requirement is to ensure Marines deploy with an operational understanding of the local military and partner cultures and regional dynamics relevant to the mission with select Marines being language-enabled, in order to facilitate mission success. The Marine Corps Center for Advanced Operational Culture Learning (CAOCL) heavily relies on contractor support to ensure Marines are equipped with operationally relevant regional, culture specific, culture general, and foundational language instruction to support effective planning and operations in the joint and combined expeditionary environment. CAOCL requires a comprehensive, flexible, fully integrated training, education, development, execution, maintenance, research, and cultural advisor capability to respond to emerging global USMC requirements and changing situations such as emerging threats and opportunities across the globe.

EXCEPTIONAL
Performance meets contractual requirements and exceeds many to the Government’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
VERY GOOD
Performance meets contractual requirements and exceeds some to the Government’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
SATISFACTORY
Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
MARGINAL
Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor’s proposed actions appear only marginally effective or were not fully implemented.
UNSATISFACTORY
Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
