STATEMENT OF WORK

PERFORMANCE WORK STATEMENT

ROMAN CATHOLIC PRIEST SERVICES

MARINE CORPS AIR STATION, CHERRY POINT, NC

1.0 SCOPE:

The purpose of this contract is to provide Roman Catholic Priest services in support of the mission and objective of the Commanding Officer’s Religious Ministries Program under the guidance of the Command Chaplain. The primary element of these services is to provide pastoral care services to the Roman Catholic military members, dependents and all other authorized personnel aboard Marine Corps Air Station (MCAS), Cherry Point, NC.

2.0 APPLICABLE DOCUMENT(S):

3.0 CONTRACTOR QUALIFICATIONS:

3.1 Must be a U.S. citizen or permanent resident alien.

3.2 Must be able to read, write, and articulate the English language fluently.

3.3 Must have a Masters in Divinity or Masters in Theology from an accredited seminary.

3.4 Must have a certificate of ecclesiastical endorsement and have chapel or parish experience.

3.5 Must be in good standing with a Diocesan Bishop or Religious Community.

3.6 Must have approval from the Archbishop for the Military Services, USA.

3.7 Must demonstrate reliability and good relational and communication skills.

3.8 Must have excellent knowledge of doctrines, ideas, liturgy, sacraments, church history, customs, practices, and organization of the Roman Catholic Church.

3.9 Must possess managerial, organizational and leadership ability commensurate with responsibility for administration of a complete religious education and sacramental preparation programs.

3.10 Must be flexible and able to adjust to short notice demands.

3.11 Must possess a working knowledge of Microsoft Word, Excel, Power Point, and Outlook or equivalent software and desktop computer hardware.

4.0 TASKS :

4.1 The Contractor shall conduct Catholic Mass every Sunday.

4.2 The Contractor shall conduct Weekly Mass every Tuesday, Wednesday, and Thursday.

4.3 The Contractor shall conduct Mass every first Friday of each month.

4.4 The Contractor shall conduct Mass on all Holy Days of Obligation.

4.5 The Contractor shall conduct confession for an hour prior to Sunday Mass and a half hour after Sunday Mass.

4.6 The Contractor shall conduct First Communion and Confirmation services as required.

4.7 The Contractor shall perform sacramental and emergency ministrations including baptism, marriage, funeral and memorial services, and anointing of the sick.

4.8 The Contractor shall prepare sacramental records.

4.9 The Contractor shall compose and publish a weekly bulletin that contains, at a minimum, liturgy information, ministries, and special events throughout the year.

4.10 The Contractor shall compose and publish the “Prayers of the Faithful” for Sunday and Holy Day Liturgies in accordance with Liturgical guidelines and Command Religious Program needs.

4.11 The Contractor shall maintain liaison with the Archdiocese for the Military Services, USA.

4.12 The Contractor shall provide emergency counseling and spiritual support, as required, within two (2) hours of notification, for the following, but not limited to: last rites, birth or stillborn child, etc.

4.13 The Contractor shall coordinate musical support with the assistance of the Roman Catholic Music Director and/or Musician.

4.14 The Contractor shall train and certify liturgical ministers (lay Eucharistic ministers, hospitality ministers, and altar servers, etc.).

4.15 The Contractor shall coordinate and provide spiritual educational classes for the Catholic Religious Education Program.

4.16 The Contractor shall inform the Command Chaplain of any problems and concerns in regard to morale and Catholic services and programs.

4.17 The Contractor shall meet with the Command Chaplain once a month to discuss any issues pertaining to contract requirements.

4.18 The Contractor shall submit a Monthly Status Report (MSR) via e-mail to the Command Chaplain, which provides information on work completed, work in progress, work outstanding, in addition to reporting problems encountered and anticipated.

4.19 The Contractor shall prepare a “Lessons Learned” document within 5 days of any problem resolution, as requested by the Command Chaplain.

4.20 The Contractor shall arrange for coverage of all contractual requirements by providing the name and credentials of a suitable replacement 15 days prior to scheduled time-off, vacation or other occasions of unavailability. Replacements must be approved by the Command Chaplain and Archdiocese for the Military Services in order to meet this requirement.

4.21 The Contractor shall be responsible for subcontractor compensation or replacement compensation.

5.0 GENERAL INFORMATION

5.1 PERIOD OF PERFORMANCE (POP): The period of performance shall be for 12 months The POP reads as follows:

30 September 2014 – 31 August 2015

5.2 RECOGNIZED HOLIDAYS: Normally services are not required during Federal holidays; however, the Contractor shall provide services on Federal holidays listed below, only as required.

5.3 HOURS OF OPERATION: The Contractor is responsible for conducting business, between the hours of 0800 -1200 and 1300 – 1600, Monday thru Friday, except when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. Contractor shall keep in mind that the stability and continuity of services are essential. Additionally, the Contractor shall complete all tasks (section 4.0) whether during, before, or after normal hours of operation at no additional cost to the Government.

5.5 PLACE OF PERFORMANCE: The work to be performed under this contract shall be performed primarily at the Chaplain’s Office and facilities aboard or near MCAS, Cherry Point.

5.6 TYPE OF CONTRACT: The Government will award a firm-fixed price contract.

5.7 PERFORMANCE REVIEWS/ PERIODIC PROGRESS MEETINGS: The COR will conduct a performance review of the Contractor on a quarterly basis. Contractor performance will be evaluated in accordance with contract requirements and the Performance Requirements Summary (section 11.0). At these meetings the Command Chaplain will apprise the contractor of how the government views the contractor's performance and the contractor will apprise the Government of problems, if any, being experienced. Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the government.

6.0 GOVERNMENT FURNISHED EQUIPMENT:

6.1 The Government will provide adequate working space, all equipment, (e.g. desk, chair, computer, printer, copier, telephone, etc.), utilities, janitorial services, and expendable operating supplies.

6.2 The Government will provide telephone and facsimile usage for the performance of official government business.

6.3 The Government will provide professional resource material and celebration of sacrament supplies.

7.0 INSPECTION AND ACCEPTANCE:

Inspection and acceptance of the services to be furnished hereunder shall be made in accordance with the Performance Requirements Summary specified in paragraph 12.0.

The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records--Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

(e) (1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c), and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1)in a subcontract for commercial items. Unless otherwise indicated below, the extent of the flow down shall be as required by the clause—

Section L - Instructions, Conditions and Notices to Bidders

1) SUBMISSION OF QUOTES

a) The vendor is responsible for ensuring that quotes are received in the Contracting Office no later than 12:00 P.M. (EST) on 22 August 2014 as indicated on page one (1) of this solicitation. Failure of vendor’s quote to be received prior to the hour and date specified may result in rejection of the quote.

b) Quotes shall be submitted electronically via fax to 252-466-4056 or e-mail to jayne.bell@usmcmil.

c) RFQ Number M00146-13-T-2577 must be clearly shown in the subject line.

2) INSTRUCTIONS FOR THE PREPARATION OF QUOTES: The vendor shall submit past performance, and price information in accordance with the following instructions:

a) Quote Instructions:. The vendor’s quote shall describe in detail and provide information that demonstrates how their quoted service meets or exceeds the minimum requirements of the Purchase Statement of Work. The required Period of Perfromance is 30 Sep 2014 thru 29 Aug 2015.

b) Past Performance Information Instructions

i) Vendors shall submit past performance information requested below as part of their quotation.

ii) Vendors shall submit at least there (3) relevant past performance references to the Contracting Office with their quote. Deadline for submittal of past performance information is the same as the deadline for submittal of the entire quote. Failure to provide the requested information will render the quote incomplete and may exclude it from further evaluation. If a vendor has no relevant past performance history, the vendor must affirmatively state that it possesses no relevant past performance history. Vendors with no relevant past performance history available will be evaluated neither favorable nor unfavorably on past performance.

iii) Relevant past performance is defined as performance on contracts of similar in scope/complexity and dollar value as this requirement performed within the past three (3) years. For the purpose of the RFQ, “similar in scope/complexity” means delivery of similar performance of services similar to those required by this RFQ. Reference information shall include agency or company name, contract number(s) if applicable, point of contact information (to include e-mail address and phone number), brief description of the supplies provided, and contract dollar amount.

c) Price Quote Instructions: The vendor’s price quote shall include:

i) The vendor’s proposed firm-fixed price for each line item shown in Section B. Vendor must include unit price and total amount for each line item.

ii) The vendor shall quote FOB Destination pricing.

d) Representations and Certifications Instructions. Vendors are to include completed copies of the Representations and Certifications incorporated into Section K of this document by reference and in full-text with their quote. Section M - Evaluation Factors for Award

BASIS OF AWARD

a) The Government expects to award a firm-fixed price purchase order as a result of this solicitation to the lowest-priced technically acceptable vendor who demonstrates a satisfactory past performance record. Award will be made on an all or none basis.

b) Evaluation of quotes will be conducted on a low-priced, technically acceptable basis. All offers will be evaluated first on price. The lowest priced offer will be evaluated for technical acceptability on an acceptable/unacceptable basis. A technically acceptable offeror is one that demonstrates their ability to perform the services in accordance with the requirements of the Statement of Work.

c) If found to be technically acceptable, past performance will be evaluated. Acceptable past performance is a record of satisfactory performance on other relevant contracts. If past performance is found to be satisfactory, then no further evaluations will be conducted and award will be made. If the lowest priced offer is not technically acceptable with satisfactory past performance, the next lowest priced offer will be evaluated and so on, until a technically acceptable offer with satisfactory past performance is reached.
