LTV Preliminary Hazard Assesement August, 2022

There are standard hazards that should be assessed such as structural failure, loss of control, fire/explosion, failure of hardware/software to perform, separation hazards, debris hazards, failure of the communication system(s), etc. The safety analyst should assess the systems, subsystems, and functions of the hardware/software and evaluate the interaction with other systems during various phases of the hardware/software development, manufacturing, assembly, pre-launch, launch and space flight operations.

This document does not contain information which falls under the purview of the U.S. Munitions List (USML), as defined in the International Traffic in Arms Regulations (ITAR), 22 CFR 120-130, nor the Export Administration Regulation (EAR), 15 CFR 730-774, and is not export controlled. This publication is not intended for distribution outside of the NASA and International Partner communities. Release to the public approval
