About this file

This is a Compliance Certification Form for 13 C.F.R. 125.6, pertaining to Solicitation Number 36C24126Q0641. The form requires offerors to certify their compliance with federal subcontracting limitations applicable to small business contracts. Offerors must select one of two compliance options: either certify that no subcontractors will be used and the prime contractor will perform 100% of the contract work, or certify that the prime contractor and similarly situated subcontractors will collectively perform at least 50% of the contract value. For the second option, similarly situated subcontractors must meet the size standards for the NAICS code assigned by the prime contractor and must be certified in the Vendor Information Pages (VIP) database.

If subcontractors are anticipated, offerors must provide detailed information for each subcontractor, including business name, DUNS number, description of work, and percentage cost of contract services. The form requires calculation of the total percentage of contract value performed by the prime contractor and the total percentage performed by similarly situated subcontractors. The certification warns of severe penalties for violations, including fines up to the greater of $500,000 or the amount spent in excess of permitted subcontracting levels, and potential debarment. The Contracting Officer may require contractors to demonstrate compliance at any time during contract performance by providing invoices, subcontracts, or task value documentation. The completed certification must be signed by the contractor and returned via email to the Contracting Officer.
