About this file

This is a Minority Business Enterprise (MBE) Form issued by the City of Fort Myers pursuant to Resolution 38-125, establishing documentation requirements for contractors bidding on local projects. The form serves as a standardized template for both prime contractors and subcontractors to demonstrate compliance with the city's 15% MBE participation goal. Respondents are required to complete separate forms for each MBE prime and subcontractor involved in the project, documenting the specific work to be performed, project commitment and completion dates, and the dollar amounts allocated to MBE participation.

The form requires contractors to document good faith efforts across six distinct steps: prime contractor and MBE information submission including total contract amounts and MBE dollar allocations; transmittal letters with certified mail receipts verifying outreach to MBE firms; telephone and in-person visit records with dates and contact names; follow-up documentation on incomplete or non-responsive quotations with specific explanations of deficiencies; detailed descriptions of work performed by MBE contractors; and explanations for any selected MBE bids that were not ultimately utilized, including identification of alternative subcontractors and their contract amounts. This pragmatic format enables contractors to systematically demonstrate their demonstrated good faith efforts in achieving the established MBE participation goal through documented outreach, communication, and consideration of qualified minority-owned firms.
