About this file

This is a Minority Business Enterprise (MBE) Form document issued by the City of Fort Myers under Resolution 38-125, designed to document good faith efforts in achieving MBE participation goals on local contract opportunities. The form serves as a standardized reporting mechanism for prime contractors and subcontractors to demonstrate compliance with the city's 15% MBE participation goal. The document requires respondents to provide detailed information across six steps: prime contractor and MBE contractor information, transmittal letter records showing outreach to MBE firms via certified mail or alternative delivery methods, telephone and in-person visit documentation with specific dates and times, follow-up records on incomplete or non-responsive MBE quotations, and explanations for any MBE firms that submitted bids but were not ultimately utilized on the project. Contractors must attach copies of transmittal letters sent to MBE firms along with certified mail receipts or proof of delivery.

The form emphasizes the requirement that good faith efforts be specifically tailored to the work items intended for subcontracting and matched to the availability and capability of applicable MBE firms. Prime contractors and their representatives must document all communication with MBE firms, including the nature of any deficiencies in submitted bids and the substance of follow-up discussions. For any MBE bids that are selected but not utilized, contractors must provide detailed explanations and identify the alternative subcontractors selected for that portion of work, including the amounts awarded to those alternatives.
