About this file

The document is a Minority Business Enterprise (MBE) Form for the City of Fort Myers, Florida, associated with a brick paver maintenance and installation project. The form is designed to document a contractor's good faith efforts to meet the city's 15% MBE participation goal, requiring detailed documentation of outreach to minority-owned businesses, including transmittal letters, contact records, and explanations for any non-utilized or non-responsive MBE quotes. The form provides a structured approach for prime contractors to demonstrate their commitment to engaging minority businesses in the project.

The form captures critical information about potential MBE prime and subcontractors, including company details, contract amounts, scope of work, project dates, and communication efforts. It includes six distinct steps for documenting minority business engagement: prime contractor information, MBE contractor details, record of transmittal letters, telephone/in-person visit documentation, handling of incomplete quotations, and explanations for non-utilized MBE firms. This comprehensive approach supports the City of Fort Myers' commitment to promoting diversity and inclusion in municipal contract opportunities, specifically for a project involving brick paver maintenance and new installation across various urban surfaces.
