About this file

This document is a certification form regarding limitations on subcontracting for a solicitation from the Department of Veterans Affairs Veterans Health Administration. Offerors must certify whether any subcontractors are anticipated in performing janitorial services under contract number 36C24222Q1046 for janitorial services at the Northport VA Medical Center. If subcontractors are anticipated, offerors must identify each subcontractor, provide their DUNS number and description of work, and state the percentage of contract work and total value of the contract each subcontractor represents. Additionally, offerors must certify that if subcontractors are used, the prime contractor and any similarly situated subcontractors will receive at least 50% of the total value of the contract. The certification warns of severe penalties for contractors found to be in violation of limitations on subcontracting under the Federal Acquisition Regulation during contract performance.
