This document is a Request for Quotes (RFQ) issued by the Ohio Department of Developmental Disabilities (DODD) to award a contract for Legionella water testing and disinfection services at eight developmental centers across Ohio. The RFQ outlines the purpose to collect water samples, perform lab testing, and provide disinfecting services to prevent the spread of Legionnaires' disease. The RFQ provides the schedule, including an inquiry period, site visits, and the due date for quotes of October 21, 2024. The contract is planned to commence on November 1, 2024 and include two routine semi-annual testing cycles, with an option to renew for up to two additional years.

The RFQ details the required services, including the number of estimated water samples per testing cycle, the specifications for sample bottles and collection, and the requirements for the testing laboratory. It also outlines the disinfection services required if sample results exceed 5 CFU. The cost proposal must include per sample testing rates, hourly labor rates for disinfection services, markup percentages for chemicals/parts, and any additional charges. The contract will be awarded to the lowest responsive and responsible bidder based on the evaluation criteria.
