About this file
This file contains Legal Disclosure Instructions issued by the City of Harrisburg Office of Purchasing for a Line Striping and Pavement Markings project in Pennsylvania. Responding firms must submit a signed self-certification letter disclosing relevant legal matters and compliance history. The instructions require disclosure of fines and disbarments from governmental agencies such as the EEOC and Department of Labor related to the proposed work, as well as details of any professional disbarments. Additionally, firms must disclose outstanding legal disputes with municipalities, including the City of Harrisburg, and provide a comprehensive list of all judgments and ongoing litigation relative to the services proposed over the last five calendar years. All information must be presented in letter format, double-checked for accuracy and completeness, and signed by an authorized representative affirming the truthfulness of the disclosures.
Responding firms must also disclose any contracts for the services proposed that were terminated early during the past five calendar years, including the specific reasons for termination. The City of Harrisburg requires that all legal disclosure information be compiled, verified, and submitted according to the specifications outlined in the RFP. Interested prime and subcontractors are instructed to review all supporting documentation and project details prior to submitting inquiries or attending site visits, and to indicate their intent to bid by logging into the project profile on PennBid, which serves as the sole source of accurate information regarding the bid requirements and specifications.
