About this file

This document is a Questions and Answers addendum for an Invitation for Bid (IFB) issued by California Correctional Health Care Services for Leadership Training (RFP #SD15-00026) dated April 6, 2016. The contract involves delivering 20 Leadership Challenge Workshops between April 29, 2016 and April 27, 2018, with workshops to be scheduled on separate dates throughout the contract period. All trainers must be Leadership Challenge Certified Master Facilitators and Certified Leadership Practices Inventory (LPI) Coaches, with no alternative training programs permitted.

The document clarifies several administrative requirements, including that contractors must be in good standing with the California Secretary of State and possess a valid business license, regardless of their state of incorporation. Two forms - the Security Awareness Understanding and Accountability Form (SAUA) and Form 700 (Statement of Economic Interests) - are not required with the IFB response but may be required upon contract award. The document contains the logos of California Correctional Health Care Services and features links to relevant state websites for business registration and form submission.

Invitation for Bid (IFB)

Leadership Training

RFP #SD15-00026

April 6, 2016

QUESTIONS/STATEMENTS AND ANSWERS

1. Question(s)/Statement(s):

Do we as consultants have to be certified and use The Leadership Challenge Workshop or can a proven and even better alternative program be delivered if it meets the same desired training outcomes?

Answer(s)/Statement(s):

Section I. General Information, Provision G. Bidder Minimum Qualifications states the following: “All trainer(s) shall be Leadership Challenge Certified Master Facilitators and Certified Leadership Practices Inventory (LPI) Coaches.” Therefore, the consultants have to be certified and use The Leadership Challenge Workshop.

2. Question(s)/Statement(s):

I see that there is a requirement for being certified and experienced with the Leadership Challenge Workshop. While I have extensive experience designing and delivering leadership development programs, I am not certified with the Leader Challenge method. Is this a strict requirement?

Answer(s)/Statement(s):

Please see the answer to Question #1.

3. Question(s)/Statement(s):

On page 8 it states the bidder shall be licensed to do business in California. We have done business there in years past, but are not sure what requirements you have for licensing. If you could provide guidance, we would appreciate it.

The Contractor must be in good standing with the California Secretary of State and have a valid business license. To search for your company’s status with the California Secretary of State, navigate to http://kepler.sos.ca.gov/. For more information regarding how to file with the California Secretary of State, navigate to http://www.sos.ca.gov/business-programs.

4. Question(s)/Statement(s):

You have stated there would be a total of 20 workshops between April 29, 2016 and April 27, 2018. Do you know at this time if any of those workshops will be held on the same dates or will all 20 be spread out over the two years? This will determine if we engage other master facilitators in our bid.

The workshops will be scheduled on separate dates over the course of the Contract.

5. Question(s)/Statement(s):

We are incorporated in Ohio. Do we need to be in the California Secretary of State’s Office registry?

Yes, the Contractor must be in good standing with the California Secretary of State. Please see the answer to Question #3 for more information.

6. Question(s)/Statement(s):

Attachment 12 (IT Services Division Security Awareness, etc.) seems to be for employees.

Should we just sign the form?

Answer(s)/Statement(s):

The Security Awareness Understanding and Accountability Form (SAUA) is not required to be submitted in your IFB response. The SAUA must be submitted by all trainers upon contract award.

7. Question(s)/Statement(s):

The link to Attachment 11 (Statement of Economic Interests, Form 700) did not work. I did find Form 700 on your website, but it seems to apply to those who hold public office in the state of California and I truly don’t know how to respond to the form, especially since our company and its owner reside in Ohio, and he holds no public office. Can you please advise if this form is necessary for us to complete?

Answer(s)/Statement(s):

The form is not required to be submitted with the IFB response, but may be required upon contract award. The Form 700 can be found at http://www.fppc.ca.gov/Form700.html. All applicable areas of the Form 700 must be completed.
