About this file

This is a lawncare service evaluation checklist document issued by the Department of Defense, Veterans and Emergency Management—Military Bureau for assessing contractor performance on lawn maintenance services at various state facilities including the Brunswick Armory. The checklist establishes performance standards for general lawn care tasks such as mowing grass to specified heights between 3.5 and 5 inches, trimming and edging, debris removal, and maintaining landscaped areas free of weeds and trash. Specialized maintenance requirements are detailed for specific site features including filter beds, bio-retention cells, pollination beds, retention ponds, and gravel wetlands, each with distinct cutting heights and maintenance schedules. The evaluation process requires minimum monthly periodic performance tours conducted by the Program Administrator or designee, with additional evaluations scheduled via written email notification at least three business days in advance. Work must be performed during standard state working hours of 7:30 a.m. to 4:00 p.m., excluding state and federal holidays, with contractors required to maintain a published schedule and respond to all communications within one business day.

The document establishes clear performance standards with documented consequences for non-compliance. Contractors receiving four or more negative responses or "Needs Improvement" ratings during evaluation tours will be issued a two-week corrective action period, after which the contract may be terminated with thirty days' notice. Three documented poor performance evaluations result in automatic termination eligibility. Contractors may appeal termination decisions within fifteen calendar days to the Director of the Division of Procurement Services. The evaluation checklist requires documentation through photographs where applicable and mandates that all personnel on site be properly vetted. Completed evaluation forms must be submitted to Ken Merrifield at the Department of Facilities and Equipment with signatures from the facility contact, Department Facilities and Equipment contact, and vendor/provider contact.
