Offerors shall provide rationale in support of the exception and fully explain its impact, if any, on the performance, schedule, cost, and specific requirements of the solicitation. Please be aware that if any exceptions are taken to the solicitation and award is made without discussions, your proposal will be deemed un-awardable.

d. Offerors shall identify all subsidiaries by CAGE code and location which may receive future work under this IDIQ.

e. Company’s Organizational Conflict of Interest Mitigation Plan IAW the PWS. If no actual or potential conflict of interest exists, the Offeror shall state as such in this document.

f. Validity Statement: The proposal validity statement shall be valid through 265 calendar days from the proposal due date. The offeror shall be registered in the SAM.gov at the time of proposal submission, through contract award and for the duration of the acquisition. 3.2 Contract Documentation

a. The offeror’s proposal shall include a signed copy of the SF 33 (to include clause fill-ins), Sections A through K. The offeror shall complete blocks 13 through 16 and sign and date blocks 17 and 18 in Section A of the SF 33. Signature by the offeror on the Standard Form 33 constitutes an offer, which the Government may accept. In doing so, the offeror agrees to the contract terms and conditions as written in the RFP Sections A through K. Offerors shall ensure that all clauses and provisions that require “fill-in” information are appropriately completed, including the proposed prices associated with the contract line items in the Schedule of the

RFP.

b. This will be the IDIQ contract with baseline clauses and conditions. The specific prices, contract type(s), delivery requirements and period of performance will be determined at the task order level.

OCI issues include, but may not be limited to, the inability to render impartial assistance or advice to the Government, the inability to objectively perform contract work, or the unfair competitive advantage created by current or previous contractual efforts with the Government. The offeror shall submit a mitigation plan with their proposal for each applicable domain. Submission of mitigation plans are only required if the offeror is aware of potential or actual OCI issues related to this procurement.

Offerors shall provide a legible, clear, and coherent Organizational Conflict of Interest Mitigation Plan IAW template provided in Attachment 4.

Offeror’s OCI mitigation plan will not be evaluated as part of the technical evaluation;

rather, the CO will determine whether the potential awardee is awardable in light of OCI concerns, akin to a responsibility determination. This determination focuses primarily on OCI concerns relating to the Prime Contractor. OCI concerns relating to Subcontractors will primarily be addressed as part of the consent to subcontracting process, which is a matter of contract administration.

Declaration or Mitigation Plan (as appropriate) shall address each domain as applicable.

3.5 Corporate Relationships and Structures. The Offeror shall identify the structure it will use to meet Government requirements in Appendix _ to Section L, Corporate Relationships and Structures. The Offeror shall:

7) If prior Joint Venture: a. Provide documentation of prior Joint Venture to include dates Joint Venture was in effect.

MORTAG, AMY L CIV USAF AFMC AFICC/KA (ACE)

Are you saying that the offerors cannot propose any subcontractors for this effort?

BUTLER, SHATAYA N CIV USAF USAFE 700 CONS/PKA

Revised. Subcontracting relationships will be accepted.

L.4 VOLUME II - TECHNICAL EXPERIENCE

Volume II – Factor I Technical Experience shall be provided in accordance with Table 1, Proposal Organization and within the specified submittal date of the RFP.

4.1 General. The Proposal Volume shall be specific and complete. Legibility, clarity, brevity, and coherence are important. The Offeror’s proposal will be evaluated IAW Section M.3, using the Appendix A to Section L, Self-Scoring Matrix. The Offeror shall only provide relevant pages of documents used to validate Offeror HTRO Self-Scores. Using the instructions provided below, the Offeror shall provide, as specifically as possible, the actual data used to substantiate the Offeror’s HTRO Self Score.

4.2 Work Sample Cover Sheet. Offerors shall complete a cover sheet for each work sample submitted (see Appendix B Work Sample Cover Sheet). Offerors shall follow the instructions provided within the appendix and remove the instruction sheet (page 3 of 3) prior to submission of their proposal. Each work sample shall provide at least two (2) Government Points of Contact (POC). The Government may contact the Government POCs provided. The Government reserves the right to contact additional Government POCs if necessary.

4.3 Work Samples

4.3.1. The offeror shall submit a maximum of work samples (contracts/orders) which demonstrate its technical experience in the required elements, and which comply with the work sample qualifications established in 4.3.4 below. Future performance periods on an existing contract do not demonstrate technical experience and shall not be utilized as work samples, nor be factored into the total point value of a submitted sample. When referring to work samples, the terms “contract” and “task/delivery order” are interchangeable, with “contract” often used as the all-encompassing term. IDIQ contracts, or other contract vehicles for future work not yet performed, are not acceptable proof of performance related experience, and shall not be provided as work samples. Each task order will be counted as a single Work Sample. When providing documentation for this Work Sample include the contract cover pages (SF33, SF1449, etc.) for both the IDIQ contracts and all relevant subsequent task orders. Work samples may count toward multiple categories, and it is encouraged to submit work samples that can be utilized in multiple categories.

MORTAG, AMY L CIV USAF AFMC AFICC/KA (ACE)

Suggest removing the “HTRO MTTR” portion of this sentence since it is a technique utilized to evaluate the proposal, not necessarily part of the proposal title.

BUTLER, SHATAYA N CIV USAF USAFE 700 CONS/PKA

Agree - removed.

4.3.2. The work samples shall consist of only official contract documentation (including, but not limited to, Performance Work Statements, Statements of Work, Contract Data Requirement Lists [CDRLs], Government Furnished Property Lists, Subcontracts, and Invoices). The Government will accept a Statement of Fact from the cognizant contracting officer or COR to confirm missing information, should contractual documents not carry all the information necessary to substantiate the offeror's scores. The offeror shall not create any new documentation, such as project narratives, or project descriptions. For each work sample, the offeror shall also include the signed cover page associated with the contract or order. Where official contract documentation is insufficient to substantiate a Self-Rated Score, the offeror may utilize corporate time system reports as documentation. When corporate time system reports are utilized, the citation must include a signed statement attesting to the validity of the reports. Work samples shall include sufficient information to substantiate each claim. If a work sample substantiates multiple elements each element must be identified within the work sample.

4.3.3. Work samples shall be provided in their original format. Offerors shall assign each work sample an identifier (e.g., WS1-WS10). All pages of all work samples shall be numbered. In addition, the original pagination of each document shall be retained. If the original documentation within a work sample is not numbered, offerors shall add page numbers. Font, text size, and margin requirements established in section L2 do not apply to work sample submissions. Work samples shall be in English. If more than one document is provided as part of a work sample, the documents shall be combined as one file and numbered accordingly. Offerors shall include all documentation for a given work sample in a single PDF file and add to each page a unique WS identifier and page number in the margin. While this consolidation and notation may result in some confusion (such as multiple pages identified as "page 1" if the citation includes page 1 of an SF33/SF1449/DD1155, page 1 of the PWS, etc.), the result combined with the Cross Reference Matrix (Appendix C) will ensure that Government evaluators can find all documentation quickly and efficiently. The same Work Samples can be used for rows 18 and 19 of each domain’s HTRO Matrix “Performance Related Experience”.

4.3.4 Work Samples Qualifications

Work samples shall meet these minimum qualifications:

a. Shall be a US Government contract with the Federal Government to include Department of Defense (DoD).

b. Shall be a services contract, defined as a contract whose primary purpose was/is to render services in support of the US Government with a total contract value in excess of $2M performance with ongoing performance or completion within five

(5) years of initial RFP close date. Category 1 and Category 2 contracts, as defined below are authorized unless otherwise restricted in scoring criteria.

c. Shall fall into one of the following categories:

i. Category 1: Prime offeror work samples. In order to be used, the offeror shall have acted as the Prime or as a Major Subcontractor, which may include work performed as part of a joint venture. IAW DFARS 209.571-1 “Major subcontractor” means a subcontractor that is awarded a subcontract that equals or exceeds—(i) Both the certified cost or pricing data threshold and 10 percent of the value of the contract under which the subcontract is awarded; or (ii) $55 million. If a company establishes a joint venture for this contract, work of both prime companies may be utilized. Primes may also include subsidiaries or legal entities which fall under the parent corporation. In order to use a subsidiary or legal entity as a prime work sample, (1) the acquisition of the subsidiary or legal entity shall have been finalized as of the date of initial RFP issuance, and (2) objective evidence shall be included in the work sample to demonstrate that they are now a legal entity of the corporation. If these criteria are met, subsidiary or legal entity work samples will be considered prime work samples even if the work occurred prior to the company being acquired.

Category 2: Subcontractor work samples. In order to be used, the Subcontractor shall:

(1) Be identified within the Prime’s Team Structure introduced in the Executive Summary;

(2) Have acted as the Prime or Subcontractor on the contract or order submitted; and

(3) Provide a signed Teaming Agreement as required. Teaming Structure and Teaming Agreements are only required if introduced in the proposed Work sample(s).

d. Shall include at least six months of performance which shall have been within the last five years, as of the date of initial RFP issuance. For samples where all periods of performance (POP) do not meet this qualification (such as Base Year POP outside the five-year limitation and an Option Year POP within), only the POP which meet this criterion shall be utilized for scoring purposes.

e. Work samples that do not satisfy these minimum requirements will not be further evaluated by the Government to validate the associated Self-Rated Score (SRS).
