About this file

This document is a Price Sheet (Attachment 2a) from the Arizona Department of Economic Security's Division of Employment and Rehabilitation Services for a Vending Machine Purchase, Installation, and Training contract. The document appears to be part of a solicitation or contract bid process for vending machine and related equipment services across all counties in Arizona.

The price sheet covers multiple vending machine categories including cold and hot beverage machines, snack machines, combination snack/beverage machines, cold and hot food machines, frozen food machines, water/ice stations, bill changers, accessories, payment systems, and kiosks. For each category, vendors are required to provide a percentage discount off their published list price and submit website or catalog information. The document also details standardized hourly labor rates for installation and training services, with a statewide rate of $170 per hour for installation and $100 per hour for training during regular business hours (Monday-Friday, 6:00 AM to 6:00 PM). The rates are consistent across all Arizona counties, though vendors can potentially provide county-specific rates. All proposed equipment must meet specific technical requirements, including ADA specifications, MDB/DEX compatibility, and various technological features like credit card readers and guaranteed delivery systems.
