About this file
This document presents proposal requirements for KASTOW projects in Kentucky. The proposal must be submitted as a single PDF document with numbered pages that is specific to Kentucky and site-specific conditions rather than generic in nature. The document specifies a precise organizational structure that bidders must follow, including a cover page, transmittal letter, bidders' experience and qualifications, prior projects summary, contractors' committed equipment list, proposed work plan, Davis-Bacon Act requirements documentation, subcontractor identification form, cost proposal with bidder's response forms, annual required affidavit for bidders and offerors, and a signed 5% bid bond. Proposals must not include unnecessary pages such as blank sheets or section dividers, and failure to comply with these requirements may result in the proposal being deemed non-responsive.
The requirements emphasize strict adherence to formatting and organizational standards as evaluation criteria, with generic workplans and equipment lists subject to corresponding grade reductions. No information regarding pricing terms, set asides, contract duration, funding sources, or award dates is provided in this document, as it focuses exclusively on the structural and procedural requirements for proposal submission rather than substantive project or financial details.
