About this file
This is a Standards of Conduct Declaration form (Form AG-010) from the Hawaii State Judiciary that must be completed by contractors doing business with the State. The form requires contractors to declare under penalty of perjury their compliance with Chapter 84 of the Hawaii Revised Statutes (Code of Ethics), specifically addressing potential conflicts of interest related to legislators, state employees, and business relationships.
The form includes critical requirements regarding competitive bidding processes, noting that if a contractor indicates they are a legislator, employee, or business with controlling interest ("is" box checked) and the contract value exceeds $10,000, the contract must be awarded through competitive sealed bidding (Section 103D-302, HRS) or competitive sealed proposal (Section 103D-303, HRS). Additionally, the agency must post notice of intent to award and file with the State Ethics Commission. The form defines key terms including "Employee" and "Controlling interest" and requires contractor signature, name, title, and date certification.
