MEMORANDUM
Date: 06/01/2021 From: John Trebesch
To: Craig Wells, Chief of the Contracting Office, Great Plains Area IHS
Subject: Request for Approval of Justification for Other than Full and Open Competition
This is to request approval to solicit, negotiate and award a purchase order utilizing “other than full and open competition” in accordance with the Great Plains Area policy.
Please accept the following information and justification to support my request:
1. Service Unit: Fort Thompson
2. Requisition No.: IHS1429763
3. Total Estimate Cost: $63,300.00 (Include the cost for option year if applicable)
4. Justification Statement that Price is Fair and Reasonable:
This price is fair and reasonable. The facility has recently begun to experience a sewerage gas smell throughout the facility. The main service line was videoed and the exposed ductile iron pipe was inspected. It was found the main sewerage pipe was severely cracked along with several feet of the ductile iron collector pipes located in the crawl space. This piping has created this issue as recently as last August. The maintenance personnel do not have the expertise to do this work. The facility is having to temporary close and/or delay daily patient appointments to clear the facility using fans and the building ventilation systems. The severity of the sewerage gas smell is increasingly getting stronger and more intense which causing longer delays to open the facility .
5. Program Name: Great Plains Area IHS – Fort Thompson Service Unit
6. Name of Proposed Vendor and Address:
Allied Plumbing & Heating Inc.
Duns # - 007870337
7. Brief Description of Service, Supply or Equipment:
This work will remove the existing ductile iron pipe and replace it with PVC.
8. Statutory Authority: (check appropriate box)
(x ) Sole Source FAR 6.302-1, 10 U.S.C. 2304(c)(1)
( X ) Unusual and Compelling Urgency FAR 6.302-2, 10 U.S.C. 2304(c)(2)
9. Fact and Statement Supporting the Use of the exception:
The ductile iron pipe continues to fail/crack. At present, the pipe failure is creating an unsanitary situation in the facility. The vendor has the resources to complete ASAP, the contractor will proceed once approval is provided.
