MEMORANDUM FOR
DATE
FROM:
SUBJECT: Past Performance Questionnaire for Solicitation Number FA4484-16-R-0004
FOR OFFICIAL USE ONLY
Joint Base McGuire-Dix-Lakehurst Contracting Squadron (JB-MDL CONS) is in the process of selecting a contractor for the Air Advisor Academic education and training requirement. The Air Advisor Academic training requirement consists of advisor mission and cross-cultural skills. In addition, this requirement contains courseware development and maintenance.
One of the considerations in proposal evaluation is the verification of the offerors' past and present performance on contracts, which reflect the offeror's ability to perform on the proposed effort. We have referenced your contract number XXXX with our company as a candidate for providing past/present performance information. JB-MDL CONS is depending on information received from agencies and companies such as yours, which have had first-hand experience with our company, for the evaluation of our performance on the identified contract.
The areas of interest regarding our performance are summarized in the enclosed questionnaire. We sincerely request that you please complete the questionnaire and send it directly to the Contracting Office for the Air Advisor Academic solicitation by the date set for receipt of proposals, 20 November 2015. This schedule will allow JB-MDL CONS sufficient time to analyze the data prior to the start of discussions, if held.
To assist you in preparing your response and expediting your reply, the questionnaire may be filled out either electronically or by hand, and either e-mailed or faxed to the attention of Manuel Revilla. Please call (609)754-5922 prior to a fax transmission, if you have any questions, or if receipt of your questionnaire is not acknowledged. Your completed questionnaire will become a part of the official Source Selection records.
Your help is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this Source Selection. Thank you.
