4.10 Performance Monitoring and Deficiency Tracking
A. Inspection Method
The City will conduct routine inspections of all facilities using a standardized checklist to verify that janitorial services are being performed in accordance with the contract requirements.
Inspections will be conducted on a weekly basis, at minimum, and may include any areas identified in the scope of work, including but not limited to restrooms, common areas, floors, trash removal, and high-touch surfaces.
Each inspected item will be rated as:
• Acceptable (meets contract standards), or
• Deficient (does not meet contract standards)
B. Performance Measurement
Performance will be measured based on the percentage of inspected items that meet contract standards.
Performance Score = (Number of Acceptable Items ÷ Total Items Inspected) × 100
Performance thresholds shall be defined as follows:
• 84% or higher – Acceptable Performance
• Below 84% – Deficient Performance; corrective action required
The City reserves the right to require corrective action for deficiencies in critical service areas regardless of the overall performance score.
C. Major Deficiencies (Critical Failures)
Notwithstanding the overall performance score, the following conditions shall be considered Major Deficiencies:
• Failure to report for scheduled service (“no show”)
• Failure to provide required staffing levels
• Any condition posing a health or safety risk
• Failure to correct identified minor deficiencies within seventy-two (72) hours
IFB # CS-26-02 SECTION 4 - SCOPE OF WORK Page 23
Occurrence of any Major Deficiency may result in immediate corrective action, service deductions, or other remedies, regardless of the overall inspection score.
D. Deficiency Tracking and Escalation
The City will maintain a log of all identified deficiencies, including the date identified, location, description of the issue, and date corrected.
Failure to correct deficiencies within the required timeframe, or repeated deficiencies of the same nature, may result in the following actions:
1. Issuance of a written notice of non-compliance
2. Requirement for a corrective action plan
3. Assessment of service credits, if applicable
4. Termination of the contract for default, in accordance with contract provisions
Any deficiencies identified during inspection must be corrected within twenty-four (24) hours of notification.
E. Service Deductions
If the Contractor fails to meet performance standards, the City may apply the following deductions:
1. Location-Level Deduction
A deduction of five percent (5%) shall be applied to the monthly cost for any location with a performance score below 85%.
2. Overall Contract Deduction
If overall performance across all locations falls below 85%, an additional three percent (3%) deduction shall be applied to the total monthly contract amount.
3. Major Deficiencies
The occurrence of a Major Deficiency shall require the vendor to submit a Corrective Action Plan (CAP).
Failure to correct the deficiency may result in termination.
4. Non-Exclusive Remedy
These deductions do not limit the City’s right to pursue additional remedies under the contract.
IFB # CS-26-02 SECTION 5 - PRICE PROPOSAL / BID TABLE Page 24
SECTION 5 - PRICE PROPOSAL / BID TABLE
The vendor must provide their pricing electronically through the designated line items listed on the
Bid Sheet/Pricing Table via the City's e-Procurement portal on OpenGov.
Vendor Notes: The bid tables includes a “Vendor Notes” column for any additional comments regarding the requested line item(s). A comment is preferred in the “Vendor Notes” column. If the vendor does not need to submit any comments, they may leave it blank or enter N/A or similar.
