About this file

This is a Justification for Other Than Full and Open Competition (J&A) document for a contract ceiling increase under the U.S. Department of State's HIV/AIDS Expedited Procurement Procedures authority.

The document authorizes an increase to the contract ceiling for contract number 72066821C00001 with IntraHealth International to support the Advancing HIV & AIDS Epidemic Control (AHEC) project in South Sudan. The action is necessary to accommodate the consolidation of two legacy Centers for Disease Control and Prevention (CDC) programs—ICAP (Columbia University) and CMMB (Catholic Medical Mission Board)—into the existing AHEC mechanism. ICAP transitions to AHEC management on April 1, 2026, followed by CMMB on October 1, 2026, bringing approximately 45,000 Persons Living with HIV under AHEC's care. The services provided include antiretroviral therapy, patient monitoring, community health worker support, and laboratory services. The performance period runs from November 23, 2020, through September 30, 2028. The Department of State justified the non-competitive approach based on 40 U.S.C. 113, as implemented by AIDAR 706.302-70(b)(3)(ii), and the HIV/AIDS Expedited Procedures Package authority, determining that competition would impair foreign assistance objectives during this critical transition. The current TEC previously stood at an unspecified amount; Modification 19 added funding for October 1, 2025, through September 30, 2028. No synopsis was published under FAR 5.202(a)(11), and no formal market research or new sources were solicited due to the urgent, time-sensitive nature of maintaining uninterrupted HIV services. The Contracting Officer determined the proposed pricing to be fair and reasonable based on comparison to current pricing under the existing contract and review of the contractor's Negotiated Indirect Cost Rate Agreement.

Mission Board)—are being transitioned and absorbed into the existing Advancing HIV & AIDS Epidemic Control (AHEC) project.

This consolidation requires an immediate increase in the contract ceiling to account for the expanded geographic scope, increased patient load, and the integration of technical staff from the legacy CDC projects.

Without this funding adjustment, the AHEC project would lack the resources to maintain the high-volume clinical services and laboratory support currently provided by ICAP and CMMB, resulting in a catastrophic gap in the national PEPFAR response.

This ceiling increase will ensure the continuation of life-saving HIV/AIDS services, including antiretroviral therapy (ART), patient monitoring, and community health worker support, during the transition of program oversight from CDC to the Department of State (DOS) through September 2028.

There is no Period of Performance extension, this J&A is solely to capture an increase to the contract ceiling.

This action is critical to prevent a lapse in services that would pose an immediate risk to public health and human life in South Sudan.

SECTION 3: DESCRIPTION OF SUPPLIES/SERVICES REQUIRED TO MEET THE AGENCY’S

NEEDS

The Advancing HIV & AIDS Epidemic Control (AHEC) project, implemented by IntraHealth, provides essential HIV/AIDS services in South Sudan, including antiretroviral therapy, patient monitoring, and community health worker support. These services are critical to maintaining uninterrupted care for a large population of HIV-positive individuals.

With the ongoing transition of HIV programming from CDC to the Department of State, an adjustment to the Total Estimated Cost (TEC) and funding ceiling is required to ensure AHEC has the necessary resources to absorb expanding clinical responsibilities. Previously, funding for the ICAP and CMMB programs was managed through the CDC. As these programs transition to DOS oversight, those resources must be redirected to the AHEC mechanism to maintain operational continuity.

This funding increase is critical for AHEC to successfully take over ICAP on April 1, 2026, followed by CMMB on October 1, 2026. This strategic expansion will bring nearly 45,000 Persons Living with HIV under AHEC’s care. Without a ceiling increase to account for these two additional programs, AHEC will lack the budgetary authority to sustain life-saving services for this expanded patient population. This poses an immediate and unacceptable risk to public health and the integrity of U.S. foreign assistance in South Sudan.

The proposed adjustment seeks to increase the Total Estimated Cost of the AHEC mechanism from to a new ceiling of , representing an increase of . This adjustment accounts for the assumption of the ICAP and CMMB programs and the clinical care of approximately 45,000 Persons Living with

HIV.
