About this file
This is a Justification for Other Than Full and Open Competition (J&A) for a contract extension with the Department of State's Office of Acquisitions Management for HIV/AIDS programming in Mozambique.
The Department of State seeks to extend contract 72065619C00008 with Abt Global for 29 months (April 18, 2026, through September 17, 2028) to continue the Efficiencies for Clinical HIV/AIDS Outcomes (ECHO) project. The ECHO project provides essential HIV/AIDS services across four Mozambican provinces (Sofala, Manica, Tete, and Niassa), serving 385,569 people living with HIV, including 250,514 women and 135,055 men. Services include antiretroviral therapy, patient monitoring, diagnosis, prevention and treatment of opportunistic infections including tuberculosis, screening, referrals, treatment adherence support, and community health worker engagement across 150 health facilities. The current contract expires April 17, 2026. The extension is justified under AIDAR 706.302-70(b)(3)(ii) based on the ongoing transition of HIV programming from USAID to the Department of State, which would impair foreign assistance objectives if full competition were pursued. Abt Global is identified as the only contractor with the integrated service delivery model, established relationships with national and provincial health authorities, trained local staff, community health worker networks, functioning supply chains, financial controls, and sensitive patient database management capabilities necessary to execute this extension without service disruption. The justification notes that formal market research supporting new competition has not been conducted due to the transition timeline constraints, though ongoing engagement with Mozambique's Ministry of Health, provincial directorates, civil society, and international partners confirms strong local organizational capacity exists for potential future competition. The Contracting Officer has determined the anticipated cost is fair and reasonable based on the original award pricing analysis and historical pricing for comparable services. Specific pricing details are redacted in the document.

services such as antiretroviral therapy, patient monitoring, diagnosis, prevention, and treatment of opportunistic infections, including tuberculosis, while supporting screening, referrals, treatment adherence, and community health worker engagement.
The current contract expires on April 17, 2026. The proposed extension is for 29 months, covering the period from April 18, 2026, through September 17, 2028.
SECTION 3: DESCRIPTION OF SUPPLIES/SERVICES REQUIRED TO MEET THE AGENCY’S NEEDS
The Efficiencies for Clinical HIV/AIDS Outcomes (ECHO) project, implemented in Mozambique across Sofala, Manica, Tete, and Niassa Provinces, provides essential HIV/AIDS services that are vital to advancing epidemic control and improving public health outcomes. These services include antiretroviral therapy (ART), patient monitoring, diagnosis, prevention, and treatment of opportunistic infections such as tuberculosis, as well as screening, referrals, treatment adherence, and community health worker engagement. Together, these interventions ensure uninterrupted care for a large population of HIV-positive individuals, safeguarding lives and supporting Mozambique’s national HIV and TB response.
The current contract expires on April 17, 2026. With the ongoing transition of HIV programming from USAID to the Department of State, contract extension is therefore necessary to avoid disruption of life-saving services and to uphold the integrity of U.S. foreign assistance.
This action will authorize an extension of contract 72065619C00008, with an estimated value for the proposed 29 months of
Total Estimated Value:
Current Ceiling:
New Proposed Ceiling:
This extension will support the consolidation of the Department of State’s institutional capacity to effectively manage PEPFAR programming following the completed transition from USAID. As DOS continues to enhance its in-country, programmatic, and contracting capabilities, the extension ensures the uninterrupted delivery of critical HIV/AIDS services, safeguarding public health and maintaining program stability.
This extension is being pursued under AIDAR 706.302-70(b)(3)(ii), as competition at this time would impair the effective implementation of foreign assistance programming. Also, throughout the extension period, the contractor will be performing “more of the same” work originally contracted for, and in accordance with ADS 302.3.4.5, these changes “reasonably could have been anticipated.” The COR also confirms that the contractor has been performing at a satisfactory level.
