About this file
This is a Past Performance Questionnaire (PPQ) template used by the Library of Congress to evaluate contractor suitability for digital collection web harvesting services under solicitation 030ADV26R0010. The questionnaire is designed to be completed by current or previous customers of bidding contractors and must be returned to Colleen Daly at cdaly@loc.gov with a copy to Jennifer Zwahlen at jzwa@loc.gov by noon Eastern Time on February 9, 2026. Respondents are instructed to provide honest assessments based on contractor performance using five rating levels: Outstanding, Good, Satisfactory, Marginal, or Unsatisfactory, with narrative explanations required for any Marginal, Unsatisfactory, or Not Applicable ratings.
The questionnaire evaluates contractor performance across ten dimensions including overall contract management, quality of products and services, project phase management, schedule compliance, report timeliness, subcontractor quality, key personnel competence and communication, cost performance, and any major contract terminations or rescheduling due to contractor inability to perform. Respondents must provide general company information including name, point of contact, telephone number, contract number, contract type, period of performance, dollar value, and contract description. Past performance is noted as being of considerable importance to the Library's acquisition decision for these web harvesting services.
