About this file
This is a Past Performance Questionnaire (PPQ) template designed to evaluate contractor suitability for the Library of Congress Web Harvesting Services contract (030ADV26R0010). The questionnaire must be completed by current or previous customers of bidding contractors and submitted to Colleen Daly (cdaly@loc.gov) with a copy to Jennifer Zwahlen (jzwa@loc.gov) no later than 12:00 PM Eastern on February 24, 2026. Past performance is identified as a considerable factor in this acquisition, making customer input critical to the proposal evaluation process.
The questionnaire uses a five-point rating scale (Outstanding, Good, Satisfactory, Marginal, Unsatisfactory) plus a Not Applicable option to assess ten performance areas: overall contract management, quality of products and services, project phase management, schedule timeliness and compliance, report submission timeliness, quality of subcontracted efforts, competence of key personnel, personnel communication effectiveness, cost performance, and any major contract terminations or rescheduling due to contractor inability to perform. Respondents must provide narrative explanations for any Marginal, Unsatisfactory, or Not Applicable ratings and supplement evaluations with input from others within their organization. The template requests basic contractor information including company name, point of contact, contract number, contract type, period of performance, dollar value, and contract description.
