About this file

This document is a Past Performance Questionnaire (PPQ) for a Library of Congress contract related to the National Library Service for the Blind and Print Disabled (NLS) Business Study. The PPQ is designed to evaluate a contractor's performance across ten key areas, including overall contract management, product/service quality, project effectiveness, timeliness, reporting, personnel competence, communication, and cost performance. The questionnaire uses a rating scale from Outstanding to Unsatisfactory, with specific definitions for each performance level.

The PPQ is part of a solicitation (030ADV26R0004) seeking contractor support to help NLS research a significant business model transformation. The Library of Congress is seeking assistance in exploring potential strategic changes for their national library program, which provides braille and recorded materials to individuals with print disabilities. The deadline for completing and returning the questionnaire is noon (12:00 PM Eastern) on September 29, 2025, with responses to be emailed to Colleen Daly and Jennifer Zwahlen. The evaluation aims to gather comprehensive insights into the contractor's past performance to determine their suitability for providing business consulting services to the National Library Service.

Satisfactory
Performance meets contractual requirements. The element being assessed contains some minor problems for which corrective actions taken by the contractor were satisfactory.
Marginal
Performance does not meet some contractual requirements. The element being assessed reflects a serious problem for which the contractor did not/has not yet identified corrective actions.
