About this file

This document contains a past performance rating form for federal contractors and a reference to a related federal contract opportunity. The past performance rating form is a template for assessing contractors on six criteria: quality of service, schedule, cost control, business relations, management of key personnel, and utilization of small business. For each criterion, the government customer would select an adjectival rating of exceptional, very good, satisfactory, marginal, or unsatisfactory, and provide additional comments if necessary. The form also includes spaces for identifying the contractor, contract and project details.

The related federal contract opportunity is a pre-solicitation notice for the Alliant 3 GWAC 2nd draft RFP and RFI from GSA Federal Acquisition Service. No other details are provided on required products or services, response dates, or other salient information.
