About this file
This is a Contractor Staffing Access Questionnaire (FAA Form 4400-48) required by the Federal Aviation Administration to identify access needs for potential contractor employees supporting FAA contracts.
The form requires contractors to provide the estimated total number of employees (full-time, part-time, and subcontractor personnel) that will support the contract throughout its duration, with an option to attach a detailed personnel list. For each labor category (up to three are provided on the template), contractors must specify position titles and the number of employees in that category. The questionnaire then requires yes/no responses for each labor category regarding three specific access requirements: (1) whether employees will require routine access (more than one day per week) to FAA facilities for more than 180 days; (2) whether employees will require access to FAA computer systems or networks; and (3) whether employees will require access to FAA Sensitive Unclassified Information (SUI), including building designs, drawings, research and development material, and operations plans. The FAA uses this information to determine appropriate background investigation requirements and badging needs in the event of contract award. The form is subject to OMB Control Number 2120-0595, with an estimated public reporting burden of approximately 25 minutes per response.
