About this file
This document is a Request for Quote (RFQ) issued by the Illinois Department of Veterans Affairs for the procurement of 11 Standard Sized Convertible Mattresses with Air Pumps. The full minimum specifications for the products are provided in Appendix A. Vendors must submit their full product specifications with the bid and provide in-service and demonstration training to facility staff upon product delivery. The RFQ includes a pricing breakdown template for the bidders to complete, and specifies that the winning bidder will be selected based on the lowest total bid for all products. The RFQ states that quoted prices must be valid for 180 days, and no price increases will be accepted, with the agency guaranteed the lowest price in the event of a decrease. The agency reserves the right to reject all offers, award by item or group of items, and waive minor defects. Vendors may attach a separate page describing their proposal for the equipment or services.
The RFQ notes that the use of a brand name is for describing the desired standard of quality, performance, and characteristics, and "or equal" submissions will not be rejected due to minor differences that do not affect the product's suitability. The burden of proof that a product is equal is on the bidder. The RFQ also specifies that if there is a conflict between the information provided on paper and through the BidBuy system, the paper information will represent the bidder's intended submission.
