About this file
This is an Attachment D Certified Disadvantaged Business Outreach Plan form for an Invitation to Bid (ITB S-62900-0018046) issued by the State of Oregon's Department of Administrative Services (DAS) Procurement Services for aerial seeding services on the Willamette National Forest. The project involves aerial seeding application services to reforest 5,024 acres located east of Oakridge, Oregon, under the Good Neighbor Authority. The ITB requires bidders to complete and submit this outreach plan along with all other required bid documents by the specified bid closing date and time.
The form requires bidders to disclose their Oregon Certified Firm status under ORS 200.055 classifications (Disadvantaged Business Enterprise, Minority-Owned Business, Woman-Owned Business, Service-Disabled Veteran-Owned, or Emerging Small Business), document past contractual relationships with certified and non-certified firms within the previous two years, and provide a detailed outreach plan if subcontracting opportunities are anticipated. Bidders must describe specific steps to solicit certified firms, identify subcontracting opportunities with approximate dollar values, and confirm willingness to report subcontractor identity and subcontract values to COBID (Oregon Certification Office for Business Inclusion and Diversity). The outreach plan information is not considered in bid evaluation but ensures certified firms receive equal opportunity to participate in contract performance.
