About this file
This is a Certified Disadvantaged Business Outreach Plan form (Attachment E) from DAS Procurement Services for ITB S-63500-00012253 regarding an Automated Egg Sorter procurement. The form requires bidders to certify their commitment to providing equal opportunities for Certified Firms (defined as minority-owned, woman-owned, service-disabled veteran-owned, and emerging small businesses certified under ORS 200.055 by the Oregon Certification Office for Business Inclusion and Diversity) to participate in any subcontracting opportunities.
The form requires bidders to indicate if they are an Oregon Certified Firm, specify their certification types (DBE, MBE, WBE, SDV, ESB), list their contractual relationships with both Certified and non-Certified diverse firms over the past two years, and declare if they foresee subcontracting opportunities. If subcontracting opportunities exist, bidders must describe their outreach plans, estimate dollar values of potential subcontracting opportunities, and confirm willingness to report subcontractor information to COBID if awarded the contract.
