About this file

This is an Affidavit of Compliance document required by Cedar Springs Public School District for its Security Grant request for proposal, governed by Michigan Public Act No. 517 of 2012 (Iran Economic Sanctions Act). Bidders must certify that neither they nor their officers, directors, and employees are "Iran Linked Businesses" as defined by the Act, and commit to maintaining this status throughout contract performance if awarded. The affidavit requires notarization and must be completed with the bidder's name, authorized representative signature, title, date, and notary information.

The document establishes significant compliance penalties for false certifications, including civil penalties up to $250,000 or twice the contract amount (whichever is greater), reimbursement of investigation costs and reasonable attorney fees, and ineligibility to bid for three years from the date of determination of false certification. This compliance requirement applies to all bidders participating in the Cedar Springs Public School District Security Grant opportunity and must be executed as part of the bid submission process.
