About this file
This is a compliance affidavit document issued by Lakewood Public Schools in Michigan for the Lakewood LECC, Elementary, Middle School, and High School Classroom/Large Space A/V Upgrades Request for Proposals. The document serves as a certification requirement that all contractors submitting bids must complete and notarize prior to proposal submission. The affidavit requires contractors to certify compliance with Michigan Public Act No. 517 of 2012, the Iran Economic Sanctions Act, and to confirm that neither the contractor nor its officers, directors, and employees are designated as "Iran linked businesses" as defined under the Act. Contractors must further commit to maintaining compliance throughout the contract performance period.
The document outlines significant penalties for false certification, including civil penalties of up to $250,000 or twice the contract amount, whichever is greater, plus investigation costs and attorney fees. Additionally, any person submitting false certification becomes ineligible to bid on future Requests for Proposals from Lakewood Public Schools for three years from the determination date. The affidavit requires the contractor's authorized representative to sign and have the document notarized before a Michigan notary public, including specification of the county where notarization occurs and the notary's commission expiration date.
