About this file

This document is an Iran Economic Sanctions Act Affidavit of Compliance for the Walled Lake Consolidated School District's Invitation for Bids (IFB) for the Purchase of Musical Instruments. The affidavit requires bidders to certify that they are not an "Iran linked business" as defined by Michigan Public Act No. 517 of 2012, and that they will not become an "Iran linked business" during the contract performance.

The affidavit outlines significant legal and financial consequences for submitting a false certification, including potential civil penalties up to $250,000.00 or twice the contract amount, investigation costs, attorney fees, and a three-year prohibition from bidding on proposals. The document provides a template for bidders to complete, including signature lines for the bidder's representative and a notary public, formalizing the certification of compliance with the Iran Economic Sanctions Act.
