About this file
This is a Statement of Work (SOW) for a Blanket Purchase Agreement (BPA) establishing support for the U.S. Air Force 309th Maintenance Support Group (MXSG) at Hill Air Force Base, Utah. The SOW defines requirements for contractors to provide installation, relocation, repair, and modification services for industrial plant equipment including CNC machines, HVAC systems, hydraulic equipment, robotic systems, welding equipment, test stands, and numerous other specialized machinery and control systems used in aircraft maintenance operations.
Contractors must submit detailed quotes addressing all project requirements using a standardized format that includes materials, labor (with hourly rates from BPA pricelists), subcontractor costs, training, rental equipment, fees, overhead, and other direct costs. Upon project completion, contractors must provide comprehensive documentation including fabrication drawings, source code (where applicable), operating and maintenance instructions, spare parts lists, and onsite training for operators and maintenance personnel. The SOW establishes mandatory requirements for Hill AFB compliance including security protocols (DBIDS badges, controlled area access authorization, in/out-processing procedures), operations security (OPSEC) training within 30 days, controlled unclassified information (CUI) handling, foreign national sponsorship procedures, antiterrorism awareness training, tool control and foreign object damage (FOD) prevention per AFI 21-101, environmental hazard awareness, safety program implementation with mishap reporting within one business day, manpower reporting via SAM.gov by October 31 annually, and environmental compliance. Normal work hours are Monday through Friday 0530-1600 excluding federal holidays and base closures, with contractors required to follow Air Force energy conservation policies, hazardous materials awareness training, and communication security (COMSEC) procedures when contacting DoD organizations.

After receiving a Lost Identification Letter from the issuing security office, the contractor is required to fill out an AF Form 1168, Statement of Suspect Witness Complaint, through the SFS Office located in the West side of Building 408. The contractor must then bring a lost letter report and/or AF Form 2586 to the SF Offices to be issued a new badge then return all forms to the Group SM.
The contractor shall create an Incident/Mishap report when a contract employee loses a DBIDS, CAB, or Vindicator. The Incident/Mishap report must be submitted no later than one business day. (CDRL A004).
In/Out-Processing All contractors are required to in/out-process from each respective organization regardless of if they are issued a DBIDS, CAB, or CAC.
Contractor employees transferring between contracts, groups or companies will return DBIDS or CAB and vindicator card. New credentials will be issued by the receiving organization. The contractor shall have three days to return DBIDS or CAB and vindicator card to issuing organization. NOTE: Contractor employees are required to be sponsored and escorted while on the installation until new credentials are issued.
Contractor employees will complete the same in/out processing checklist from the assigned organization. Cloth rags are considered tools and will follow the same regulations and procedures as tools.
If tools, and/or equipment are lost, the contractor will report the loss to the GM/COR no later than two hours after discovery. If tools are lost or not accounted for, then the contractor shall implement the lost tool procedures of AFMC supplement to AFI 21-101, chapter 10, section B.
Contractors performing functions in industrial areas on the center shall have a method for controlling and accounting for all tools used. All contractors working on the center shall inventory their tools at the beginning and end of each shift and at the end of each task to check for any lost or missing tools.

The contractor will follow the local procedures for reporting inclement weather as advertised on local television and radio stations.
Miscellaneous Notification of Debarment/Suspension Status. The contractor shall provide immediate notice to the CO in the event of being suspended, debarred or declared ineligible by any other federal department or agency, or upon receipt of a notice of proposed debarment from another DoD Agency, during the performance of this contract.

All documents and materials, to include the source codes of any software, produced under this contract shall be government owned and are the property of the government with all rights and privileges of ownership/copyright belonging exclusively to the government. These documents and materials may not be used or sold by the contractor without written permission from the CO. 3. Routine and recurring surveillance to ensure the safety requirements of this contract are enforced.
4. Competent personnel to provide surveillance of the Safety Plan.
5. Identification of segregated work site locations for operations that cannot be co-mingled with general industrial operations and the process for ACO approval of operations and changes at these specific sites.
6. All contractor personnel shall be trained and qualified to perform their duties safely.
7. The contractor shall include a clause in all subcontracts requiring the subcontractors to comply with the safety provisions of this contract, as applicable.

This notification requirement shall also include physiological mishaps/incidents. A written or email copy of ALL the mishap/incident notification shall be sent within three calendar days to the GR, who will forward it to OO-ALC/SE. For information not available at the time of initial notification, the contractor shall provide the remaining information no later than 20 calendar days after the mishap, unless extended by the ACO.
Mishap notifications shall contain, as a minimum, the following information:

C. General Safety Requirements:
| If the safety plan is modified, the contractor shall submit the proposed modification, in writing, to the Contract Administration Office safety representative. |
| The contractor is solely responsible for compliance with all federal, state and local laws, the Occupational, Safety and Health Act (OSHA) (Public Law 91-596) and the resulting standards, OSHA Standards 29 CFR 1910 and 1926, as applicable, Air Force Occupational Safety and Health Standards (AFOSH STD), to include DAFMAN 91-203, Air Force Occupational Safety, Fire, and Health Standards, DAFI 91-204, Safety Investigations and Reports, Contract Safety, and the protection of their employees. | Compliance with OSHA |
and other applicable laws and regulations for the protection of contractor employees is exclusively the obligation of the contractor. Note: AFOSH STDs are annotated because many of the Air Force Standards exceed the OSHA standard criteria. If a conflict is noted, the most stringent requirement takes precedence. The government shall assume no liability or responsibility for the contractor's compliance or non-compliance with such requirements. The contractor shall furnish to each of his/her employees a place of employment, which is free from recognized hazards. The contractor shall brief his/her employees on the safety requirements of this contract and on hazards associated with prescribed tasks. This contract shall in no way require persons to work in surroundings or under working conditions which are unsafe or dangerous to their health. The contractor must coordinate and perform work so as not to impact the safety of government employees or cause damage to government property. This requires providing personnel with protective equipment and associated safety equipment as may be necessary. The contractor must also protect personnel from hazards generated by the work. If the contractor employs BILINGUAL speaking employees, they must post bilingual signs and have written procedures for specific tasks in applicable languages.
