About this file
This is an Invitation for Bid form for the State of Illinois related to a Change Order Notice extending ISAC's contract with Parchment for Electronic Transcript Services. The document serves as the standard bidding template used by the State of Illinois for solicitations and requires bidders to submit completed forms in compliance with all specified requirements outlined in the solicitation documents. Bidders must refer to Section A.1 of the solicitation document for all required submission documents and understand that failure to meet stated requirements will result in disqualification. The form includes sections for bidders to identify themselves, specify items or services being bid, and declare compliance with the solicitation terms.
The form addresses Illinois Procurement Code preferences available to bidders, including the Illinois Business Preference and Domestic Product Preference, with vendors required to complete appropriate affidavits or designations to claim such preferences. Bidders may submit exceptions to the Standard Terms and Conditions by providing a redlined version with their offer, though the State reserves the right to accept or reject any proposed exceptions. The Standard Terms and Conditions that will govern any resulting contract are available on the Illinois Chief Procurement Office website. By executing this form, the authorized bidder representative submits a binding offer to the State of Illinois to perform in full compliance with the subject solicitation terms and conditions.
