About this file
This is an Intra-Service Section 7 Biological Evaluation Form used by the U.S. Fish and Wildlife Service (USFWS) to assess potential effects of proposed projects on federally-listed threatened and endangered species and critical habitats. The form is completed by grant applicants in collaboration with the USFWS Project Officer and submitted to the appropriate USFWS Ecological Services Office for Region 3. Applicants are instructed to complete as much of the form as possible, utilizing the ECOS database (https://ecos.fws.gov/ipac/) to generate a species list for their project area. The form requires documentation of the originating person's contact information, project title, service program, geographic location, and a comprehensive project description including all conservation elements, maps, and photographs.
The form's core requirement is a determination of effects analysis, which categorizes the anticipated impacts on listed species and critical habitats into three possible determinations: No Effect (requiring no ESFO concurrence), May Affect but Not Likely to Adversely Affect (requiring ESFO concurrence), or May Affect and Likely to Adversely Affect (requiring ESFO concurrence and potentially formal or conference consultation). The reviewing Ecological Services Office then provides concurrence, nonconcurrence, or recommendations for formal or conference consultation, with all determinations requiring supervisor signatures and dates. This form is a procedural compliance document associated with the F26AS00083 Aquatic Invasive Species Grants to Great Lakes States under the Great Lakes Restoration Initiative, enabling applicants to address environmental compliance requirements prior to grant award.

Project Title:
This evaluation form will be submitted by the USFWS Project Officer to the appropriate USFWS Ecological Services Office. Your assistance is requested to complete as many of the items on this form as possible because as the grant applicant, you are the most familiar with your project, project area, and potential effects on listed species. Given this is a consultation between USFWS Offices (thus an “Intra-Service Section 7”), the USFWS Project Officer/Originating Person will review (and potentially modify) any information you provide and will complete any items that you do not. To generate a threatened and endangered species list for your project area, please utilize: https://ecos.fws.gov/ipac/. If you generate a species list, please provide that with this form.
I. Service Program and Geographic Area or Station Name: Fisheries
II. Project Location. Include state and county:
III. Species/Critical Habitat. List all federally-listed, proposed and candidate species or designated or proposed critical habitat that may occur within the project area:
IV. Project Description. Describe proposed project or action, including all conservation elements. If referencing other documents, prepare an executive summary. Include map and photos of site, if possible. Attach additional pages as needed:
V. Determination of Effects A. Description of Effects. Describe how the action(s) will affect the species and critical habitats listed in item III. This includes a full description of your rationale for the Section 7 determination made in the next section.
B. Determination. Indicate the anticipated effects of the proposed project on species and critical habitats listed in item III by marking each applicable “effect” and the species or critical habitat to which it pertains (or attach a list indicating the same).
Mark all that apply:
( ) No Effect. This determination is appropriate when the proposed project will not directly or indirectly affect (neither negatively nor beneficially) individuals of listed/proposed/candidate species or designated/proposed critical habitat of such species. No concurrence from Ecological Services Field Offices (ESFO) required.
List species/critical habitat in this determination:
· Critical habitats
( ) May Affect but Not Likely to Adversely Affect. This determination is appropriate when the proposed project is likely to cause insignificant, discountable, or wholly beneficial effects to individuals and designated critical habitat. Concurrence from ESFO required.
List species/critical habitat in this determination:
· Critical habitats
( ) May Affect and Likely to Adversely Affect. This determination is appropriate when the proposed project is likely to adversely affect individuals of listed species or designated critical habitat of such species. Concurrence from ESFO required.
List species/critical habitat in this determination:
Signature (Supervisor at originating station):
Date:
Reviewing Ecological Services Office Evaluation (check all that apply):
