About this file

This is an Insurance Coverage Requirements document issued by the State of Michigan Department of Technology, Management & Budget (DTMB) for contractors providing services at DTMB locations. The document specifies mandatory insurance requirements that all contractors must maintain throughout the contract performance period. Contractors are required to provide Commercial General Liability insurance with minimum coverage of $2,000,000 for General Aggregate Limit (other than Products/Completed Operations), $2,000,000 for Products/Completed Operations Aggregate Limit, $1,000,000 for Personal & Advertising Injury Limit, and $1,000,000 for Each Occurrence Limit. Additionally, if motor vehicles are utilized to provide services or products under the contract, contractors must maintain vehicle liability insurance on owned, hired, and non-owned vehicles as required by law. All insurance policies must list the State of Michigan, its departments, divisions, agencies, offices, commissions, officers, employees, and agents as additional insureds and must contain a waiver of subrogation by the insurance company.

All subcontractors must either obtain identical insurance coverage or be included under the contractor's insurance policies, with the contractor remaining fully liable for subcontractor compliance with these requirements. Successful bidders must submit Certificates of Insurance on the standard Accord form or equivalent to DTMB-Financial Services-Operations Division, Delegated Procurement Unit prior to Purchase Order issuance. The certificates must clearly identify DTMB-Financial Services-Operations Division, Delegated Procurement Unit, and the service location for proper filing. Failure to provide required certificates of insurance prior to Purchase Order issuance will result in the award being given to the next competitive bidder. All insurance coverage required under the contract is designated as primary and non-contributing to any comparable liability insurance carried by the State.
