About this file
This document is a comprehensive insurance requirements matrix for various service categories developed by a county risk management department. The matrix provides detailed insurance coverage specifications for multiple service sectors including construction, business and professional services, entertainment and event services, vehicle and transportation services, and product purchases. It outlines specific insurance requirements such as general liability, automobile liability, workers compensation, professional liability, cyber liability, umbrella liability, property coverage, and environmental/pollution liability for numerous subcategories of services.
The document serves as a standardized guide for vendors and contractors, specifying minimum insurance limits for different types of services. Key details include general liability coverage typically set at $1,000,000 per occurrence, with variations based on service type and risk level. Notable requirements include cyber liability for IT services involving data access, environmental liability for services potentially involving hazardous materials, and specialized coverage for high-risk activities like special events with carnival rides or performances involving audience participation. The matrix provides flexibility for risk management to adjust requirements based on specific project scope and potential exposures, with a recommendation to contact risk management for precise coverage determination in unique or complex service scenarios.

SERVICES
Standard Insurance Requirements
General Liability Insurance
General Liability is required for all categories where a limit is shown. Coverage must be verified on the certificate of insurance for these categories. In some cases it may be acceptable for certain consultants or other professional services vendors to provide evidence of Professional Liability only.
Auto Liability Insurance
Auto Liability is required for all categories where a limit is shown. Coverage must be verified on the certificate of insurance for these categories. A certificate that does not reference auto liability coverage may be deemed acceptable if the use of autos only involves driving to/from the work location(s), and is not otherwise integral to the performance of the work. For those categories where "N/A" is listed, coverage does not need to be requested or verified.
Workers Compensation Insurance
Workers Compensation coverage is to be requested for all categories indicated as "Yes" when the work is performed on County property.
Coverage must be verified on the certificate of insurance unless the organization is exempt pursuant to FL Workers Comp statutes. For any exempt vendors, an exemption certificate must be obtained and treated similarly to a Workers Comp certificate. Contact Risk Management for additional guidance. For those categories where "N/A" is listed, coverage does not need to be requested or verified.
Additional Insured Status
Additional insured status is to be obtained for all categories where General Liability coverage is required. Language indicating additional insured status must be verified on the certificate of insurance. For Transportation Services vendors, additional insured status is also required on the auto liability coverage.
Specialized Insurance Requirements
Umbrella / Excess Liability Insurance
Umbrella Liability insurance is required for all categories where a limit is shown. Coverage may also be required for certain categories listed as "N/A" if the size and/or scope of the project warrants additional limits. Contact Risk Management for additional guidance. Coverage must be verified on the certificate of insurance in the correct amounts for these categories.
Professional Liability Insurance
Professional Liability coverage is required for all categories where indicated. Coverage must be verified on the certificate of insurance for these categories. In cases where the vendor / contractor can satisfactorily document that their professional liability coverage is contained in their General Liability policy, a separate Professional Liability policy is not necessary.
There are two categories of Professional Liability coverage. One is the "standard" requirement that applies to professional services such as architects, engineers, and other consultants. The other is a technology-based requirement that includes cyber liability and is intended to be used when a consultant has access to personally identifying information (PII) or protected health information (PHI). This requirement will apply to most of our IT systems providers and may apply to certain other contracts. C y b e r l i a b i l i t y c o v e r a g e i s r e q u i r e d f o r a l l I T h a r d w a r e / s o f t w a r e , s e r v i c e s o r w e b - b a s e d s u b s c r i p t i o n s i n v o l v i n g t r a n s a c t i o n a l s e r v i c e s – e i t h e r w e e n t e r C o u n t y o r c u s t o m e r d a t a o n t h e i r s i t e , o r t h e y c o n n e c t t o C o u n t y s y s t e m s o r h a v e a c c e s s t o C o u n t y d a t a o r s y s t e m s .
Contact Risk Management for assistance in determining the need for cyber liability coverage, if you are unsure.
Environmental / Pollution Liability Insurance
Pollution Liability coverage is required for all categories where a limit is shown. Coverage must be verified on the certificate of insurance in the correct amounts for these categories. All projects involving the potential use of or exposure to hazardous materials or pollutants are to be reviewed by Risk Management to determine the specific coverage and limits required. If the hazardous materials usage and/or exposures are known prior to work commencing, coverage will be required at contract inception. If hazardous materials exposures are discovered during the project, all work related to that activity should stop until the exposures can be addressed and any coverage requirements identified.
Property Insurance
Property coverages are primarily required on construction contracts and consist of Builders Risk for projects that involve ground-up or vertical construction; Installation Floater coverage for construction on existing structures where exposures warrant the coverage; or
Stored materials Coverage for those contracts containing a stored materials clause and no Builders Risk or Installation floater requirement. All construction contracts will require a risk assessment by the Project Manager and review by Risk Management to determine the specific coverage and limits required. This step is to be completed PRIOR to any bid or agreement with the Contractor on project pricing.
Property coverage will also apply when a Contractor has County property in their care, custody and control. The specific type and amount of property will determine what type of coverage and limits are required. Contact Risk Management for additional guidance.
"Other" Insurance Coverages other than those identified above may be required under certain circumstances. If there is a coverage identified in the "Other Insurance" field, Risk Management should be contacted to determine the specific coverage and limits required.
Additionally, if the activity or scope of work involves any of the following, Risk Management should be contacted to determine the specific coverage and limits required:
1. Serving/providing of liquor;
2. Exposure to pollutants, chemicals, or hazardous materials;
3. Use of aircraft;
4. Use of watercraft
5. Events
6. Any unique purchase, service or contract/agreement that does not appear on the matrix
OTHER CATEGORIES
Instructors - Athletic Programs Standard requirements are General Liability for $1,000,000 each occurrence for all athletic program vendors.
Performers Standard requirement is to not obtain evidence of insurance for performers. If activity is a higher risk activity, as identified below, the requirement will be General Liability coverage for $1,000,000 each occurrence.
High Risk - Performances involving: Audience participation w/ injury potential; Extensive equipment set-up / stage preparations; Use of potentially dangerous props / equipment;
Special Events Standard requirements are General Liability for $1,000,000 each occurrence for special events. Limits below this amount will be acceptable for lower risk events, as determined by the Department and/or Risk Management.
Other Agreements Standard requirements for agreements such as Easements, Right of Way and Grants from the County are General Liability for $1,000,000 each occurrence. Additional coverages or limits for some types of agreements may be required based on the type and level of exposure (for example grants involving construction services). For grant agreements where the County is paying a vendor directly for services, the requirements will be based on the type of service provided. Limits below the standard requirements will be acceptable for lower risk activities, as determined by the Department and/or Risk Management. Contact Risk Management if unsure of requirements for your agreement.
PRODUCTS PURCHASES
Insurance coverage for product purchases does not need to be verified unless it meets the criteria outlined above or in the matrix. For certain products, General Liability is required to cover potential property damage or bodily injury exposures from manufacturing or product defects. Products that are purchased along with a "service component" also require coverage. A "service component" includes installation, set-up, repair and servicing, specialized manufacturing / fabrication, delivery/unloading of construction materials or heavy equipment, or other work beyond just supplying and delivering a product. This would fall under services and be subject to the requirements indicated for the particular service. In the case of specialized manufacturing / fabrication, the requirement would be General Liability coverage at the limits indicated in the matrix.
Off the shelf products from large retailers, that do not fall into the emergency, medical, personal protective equipment or safety category do not require proof of insurance. If unsure, please reach out to Risk Management.
HOLD HARMLESS AGREEMENTS
Stand-alone hold harmless agreements are no longer required for activities in which insurance coverage has been waived, unless otherwise indicated by Risk Management.
Certificates of Insurance (COIs) / Proof of Insurance A COI or other acceptable proof of insurance is required on most transactions involving purchases or services in the county, regardless of how those goods or services are procured or the payment method involved.
If Risk Management waives the insurance requirement in any matter, an email confirmation of the waiver of insurance will be provided.
