Certificates of insurance must be executed by an authorized representative of a Florida-licensed insurer and must name the City of Fort Myers as an Additional Insured on general liability and automobile liability policies on a primary and non-contributory basis for both ongoing operations and completed operations. The City's certificate holder address is 2200 Second Street, Fort Myers, FL 33901. Renewal or replacement certificates, including all required endorsements, must be provided no later than 30 days prior to expiration, with failure to do so constituting grounds for contract termination. All insurance policies must include a waiver of subrogation against the City and its officers, officials, agents, and employees. If the solicitation is funded in whole or in part by the Florida Department of Transportation or Lee County, these entities must be included as additional insureds. The contractor's insurance is primary, and all subcontractors must maintain equivalent coverage at their own expense.
